As the abuse of Tramadol continues to rise in Ghana, the Food and Drugs Authority (FDA) is joining forces with a number of sector agencies to curb the illegal distribution and sale of the drug.

According to the Chief Executive Officer of the FDA, Mrs Delese Mimi Darko, her outfit is partnering with the Pharmacy Council to boost their surveillance.

Speaking on the Citi Breakfast Show on Tuesday, Mrs Darko said a joint task force will be formed in coming days to champion that agenda.

The FDA CEO said the task force will, among other things, visit possible places that Tramadol is sold, adding that the team will also “visit chemical shops because they [chemical shops] would be obtaining some [of the product] legally, some as well may be obtained at places that we are not aware of.”

Tramadol is a pain relief drug, which according to medical experts, functions like heroin and can cause psychotic problems as well as damage vital organs in the human body if abused.

Recent surveys have shown that the drug is being abused by some youths as well as some market women, drivers and in some cases students.

The Head of Substance Abuse at FDA, Olivia Boateng said on the Citi Breakfast Show last week that FDA is intensifying its surveillance to curb the smuggling of Tramadol in the country.

According to the FDA, its investigation shows that most of the drug comes into the country through unapproved routes making it difficult for them to track its final destination.

“Our investigations revealed that there are people, who were sourcing it from unapproved routes, not from the pharmacy and hospitals. We have intensified monitoring at the ports of entry, trying to curb it through the unauthorized routes,” Olivia Boateng added.

Olivia complained that although the drug is highly regulated, the country is inundated with influx of the product which she said comes through illegal means.

She said her outfit has collaborated with the various security agencies especially those who operate along the country’s borders to block the smugglers from bringing it into the country.

“Tramadol is a restricted and a controlled drug. It has been registered as a prescription only medicine, so you can only get it at the hospital and pharmacy on a valid prescription. We have registered 50mg and 100mg indicated for managing moderate and severe pain. In our laws now, it is considered a controlled drug. They come in through Kotoka and Tema. We have also put in monitoring mechanism. We even have local manufacturers.”

“What we found when we heard of the isolated cases is that… they are those that come through the unauthorized means, drug peddling mainly. We arrested people between 14 and 16 years who were peddling it in our open market with other aphrodisiacs and other substances including tramadol,” she added.

Olivia Boateng said her outfit has also beefed up public education after reports emerged that the drug is being abused.

“We’ve done a lot of training for our security men who man these ports. We are doing a lot of collaboration with other stakeholders. We are doing collaboration with the IGP and his mean, DVLA among other stakeholders.  So we really have upped our game especially on the education and the swoop to make sure that we curb the supply and the illicit channel,” she added.

The Ministry of Health has said it intends to restrict access to the use of Tramadol, a known painkiller, to check the rising cases of abuse of the drug.

The drug, which is said to have flooded the Ghanaian market in alarming quantities, has serious addictive tendencies, according to Health Minister, Kwaku Agyeman Manu.

Responding to questions on the floor of Parliament on Thursday, the Minister said the government will also “organise swoops on illicit tramadol products on the market.”

Among other measures, he said there will be “collaboration with the pharmacy council to restrict Tramadol display on the shelves of community pharmacies by putting them under lock and key, and also ensure strict enforcement on distribution as controlled drugs.”

He also said there will be “intensified public education through Frequently Asked Questions on social media platforms, ongoing public education on tramadol use at public areas such as transport terminals, schools, marketplaces, beaches etc.”

Some of the commonly reported side effects of tramadol include pruritus, agitation, anxiety, constipation, diarrhea, hallucination, nausea, tremor, vomiting, and diaphoresis and insomnia.

Warning from Pharmaceutical Society

The Pharmaceutical Society of Ghana had already cautioned the general public on the proliferation of the painkiller.

The Society said although the drug is approved for the management of pain, the rapid increase in its use by the youth is worrying.

Even though tramadol is an approved drug for the management of pain, the strengths approved for use in Ghana by the FDA are the 50mg and 100mg oral capsules.

Tramadol, a pain relief drug used to treat moderate or severe pains, is a household word on the lips of the youth in the Upper West Region.

However, the wrongful use of Tramadol according to medical experts can cause difficulty in breathing or even death.

Some youth in the region have become addicted to the drug, administering it in large doses and mixing it with some drinks to boost their energy.

Some farmers in the area have christened the drug “Koo da nan hum” literally translating as ‘farm and buy cow.’

The drug is apparently added to energy drinks and consumed to help energise the users to work harder on their farms.

The youth, including students, reportedly also take the drug to increase their potency during sexual intercourse with their partners.

While health workers, parents, civil society organisations and the security agencies, are all worried about the abuse of Tramadol in the region, little did they know that there are more dangerous substances that are killing the youth slowly.

Whilst Tramadol use has gained notoriety among the drugs that the youth widely abuse in the region, there are more dangerous substances in the communities which are abused on daily basis, unknown to health workers and security agencies.

The smoking of substances such as nim tree and pawpaw leaves and soaking of “wee” leaves in alcohol which is consumed as breakfast, the inhaling of psychoactive substances such as petrol and turpentine among the youth to get themselves intoxicated are known to health workers in the region.

An addict’s account indicated that “the petrol makes users charge like buffalos, gaining extra energies to do more work.

“We now consume petrol like vehicles to energize us to work like engines and we also consume turpentine to make us go “high”.

This practice has however not been scientifically proven to be a contributory factor to the recent upsurge of cardio and kidney diseases among the youth.

But one of the most dangerous of these new substances the youth consume in the communities and not known to several medical experts in the region is the almighty “Daaradaara or Ogeeri” seeds. The seeds of this flower plant are classified as “mother and father” of all the substances.

“Daaradaara” is the name given to this flower plant by the Sissala, whilst the Waala and Dagaaba also call it “Ogeeri”.

“Daaradaara” literally means, “I cannot find my way clear” and “Ogeeri” means “chew and become overly gratified”.

This drought tolerant flower is widespread, but it strives well around old bins. Its seeds are now the newly found cocaine for the youth in the communities.

“Daaradaara” fruits are thorny and its seeds are like garden eggs or pepper.

Users described it or considered it as one, but the most deadly substance worse than the rest.

“If even a Goliath dares chew two seeds of “Daaradaara” it will send him down to the ground completely. Its worse form is to add the seeds to wee and smoke. Its leaves are equally deadly when boiled and treated as tea and drink.

“A user will be wrestling with trees if one challenges to do that. Chewing two or four seeds could cause the user to drink a container full of water”, an addict told this writer.

“A user is likely to terminate his or her life if there is no water near. Just as the substance passes your mouth through your throat to the stomach, it causes the entire passage to dry up at once, and the only remedy is to continue drinking water.

“A user will also decline to eat food the entire day as he or she will not feel hungry but continue to engage himself or herself at work”, the devotee added.”

“Daaradaara”, is the “David” of all the substances. According to some users, it makes them to work like robots on their farms and in all other activities they found themselves.

It makes users to behave in an extraordinary manner. But the sad things about it are that, it makes users impair in vigour very quickly. It also makes the physical frame of users look fragile and loose weight and thoughts within the shortest possible time.

“Daaradaara” has economic value to the youth in the communities. The fruits are gathered in bags and are sold to some dealers from Burkina Faso who purchase them in large quantities.

The sellers sometime exchange the “Daaradaara” fruits for motor bicycles from the buyers. The seeds are processed into powder form and used as snorts, which the youth consume on hourly basis.

On daily basis, the youth are ruining their lives and crushing early into their graves through the abuse of plant products and other concoctions. Whether knowingly or unknowingly, they are killing themselves senselessly through abuse of hard drugs, alcoholic drinks and psychoactive substances as well as plant substances.

The abuse of drugs and substances, a good segment of the youth in the region are cooking their brains with substances which cause madness in them at tender ages, and make them unproductive in their lives permanently.

At the Wa Community Psychiatric Unit at the Wa Regional Hospital, records indicated that, several users have not been reporting to the health facilities with cases of substances abuses.

Records showed that, users would wait for their situation to go worse before they would report to the hospitals. The Psychiatric Unit registered 16 cases of alcohol and other substances cases in 2016.

The Unit however listed 100 cases of psychoactive substance mostly wee and alcohol abuses in 2015. The youth is considered the live-wire of a vibrant society. It is the youth who serve as the reservoir of any nation, and the quality or otherwise of the youth, determines the progressive or retrogressive development patterns of countries or a particular region.

With the abuses of drugs and psychoactive substances among the youth, the region risks its total development efforts. This is so because the quality of the youth is important to the progress and prosperity of the region.

The region’s educational, social, economic, health and agricultural  growth among others, and indeed, all human development endeavours will be retarded. Law and order will break down, peace and discipline will suffer, while life will be meaningless to all, as chaos, confusion, unrest and anarchy will be the lot of the people.

There is no single age group of people more affected by drugs than the youth. In some ways, it looks like it is an issue everywhere: for you, your family and your friends.

“Plain and simple, try as you might, you cannot escape the issues of drugs. Nationwide, drugs affect each and every one of us, directly or indirectly: in our homes, in our families, in school, in our dormitory, in our community, town or city.”

This is why this writer feels the Upper West Region is sitting on time bomb. For others, even though they may not use drugs, they could become victims of drug-related crimes.

And, for yet others, what may have started as occasional use can turn into an addiction that presents extraordinary health concerns with potentially grave and tragic consequences.

Drug addiction is learnt primarily through association with other users or identification or fraternizing with drug oriented societies. This is reinforced by the desire to belong. Many drug users also start from a search for pleasure.

Drug addiction is a problem to the society as well as the individual. A drug addict is a prospective criminal especially in terms of prostitution and robbery. Since drug addicts have a deep craving for drugs, they commit these crimes in order to get money to buy the drugs. Drug addiction also leads to an indirect loss of talent or normal functioning abilities.

This is a worrisome situation awaiting the region, as a drug addict is not able to fulfill what society expects of him or her, and is unable to pursue a career that society values, so he or she becomes a burden on society by way of mental disorders.

The weight of consequences of drug abuse among the youth will first hit hard on parents and government, as well as health authorities, and it is hard time stakeholders collaborated effectively and come out with measures to help save the youth from the abuse of hard drugs and other substances, especially the abuse of “Daaradaara”, which is consumed at the blindside of health workers.

Upper West needs a healthy, quality and skillful youth for a progressive development, and all must work collectively and individually to protect its human resources from going into the drain.

However, just like “wee” which has its good sides and negative sides, this writer wants to challenge Ghanaian scientists to research into “Daaradaara”, maybe it could be more useful than harmful.

The Food and Drugs Authority (FDA) has seized 524,191 capsules, and 3,001 vials of Tramadol over the past eleven months.

Vial is a small glass or plastic vessel or bottle, often used to store medication in the form of liquids, powder or capsules.

The FDA said about 85 per cent of the seized drugs were manufactured in India whilst 13.5 per cent had no manufacturer details on them.

Mrs Olivia A. Boateng, Head of Tobacco and Substance Abuse Department of the FDA, made this known at a six-day training programme in Pharmaceutical Crime, Intelligence Gathering and Investigations in Accra.

She said the drugs were seized following swoops carried out by the FDA and other security agencies in three markets in Northern, Volta and Western Regions.

Mrs. Boateng said whilst some of the drugs had expired, others were not registered by the FDA, and many others had efficacy level above the FDA’s stipulated requirement.

She said over 40 million pills of counterfeited Tramadol were seized at the Port of Cotonou in Benin in 2015.

She said the FDA was collaborating with the Pharmacy Council to restrict prescription, sale and distribution of Tramadol in the country.

Mrs. Boateng said the FDA was also strengthening its ability to undertake follow-up inspection, to monitor the distribution records of manufacturers and importers of codeine-containing syrups and Tramadol in the country.

Mrs Boateng cited lack of funds, inadequate staff and logistics, as well as unauthorized points of entry as some of the challenges facing the FDA.

Despite these challenges, she said the FDA would continue to step up public education on the dangers of the abuse of Tramadol which could lead to deaths.

Tramadol is a synthetic narcotic pain reliever, and it is usually prescribed to patients suffering from moderate to severe pain.

However, its abuse has raised concerns among regulatory bodies such as the FDA, hence the effort to confiscate them.

Abuse of Tramadol could result in erectile dysfunction, infertility, brain damage among others.
